Alprostadil, sold under the brand name MUSE, which stands for Medicated Urethral Suppository for Erection, is a urethral suppository used in the treatment of erectile dysfunction. Injectible forms of this drug are sold under the name Caverject.
Alprostadil is a suppository that is inserted into the urethra at least ten minutes before sexual activity. Other companies are developing forms of alprostadil in transdermal patches and for female sexual arousal disorder. The transdermal forms are already sold in China and Hong Kong, but have not yet gained regulatory approval in the U.S.
Other forms of alprostadil must be mixed by a compounding pharmacist. Some formulas may include other ingredients such as phentolamine. Alprostadil works by causing blood vessels to dilate. When blood vessels in the penis are dilated, an erection is easier to achieve. An erection should occur about 10 minutes after insertion of the alprostadil. Patients are advised not to take more than two suppositories within 24 hours.
To take alprostadil, a man should urinate and shake excess urine from the penis. The suppository is designed to dissolve in the drop or two of urine that remain in the urethra. Insertion into a moist urethra is more comfortable than with a totally dry urethra.
For best results it is essential the penis is held upright for approximately 30 seconds after application to ensure the medication spreads sufficiently.
The main side effect for alprostadil is vaginal burning in women whose partner uses the medicine. This is usually mild, and can be prevented by use of a water-based lubricant. This drug has not been extensively tested during pregnancy, so it is advisable that couples use contraception while using alprostadil. In sex with a woman who is already pregnant, condom use is advised.
